4.2          Pabal

 

4.2.1       Indikationen

  • Fachinfo: Atonieprophylaxe bei Sectio caesarea in Epidural- oder Spinalanaesthesie
  • Alle anderen Ind.: off-label-use. Indikation in Allgemeinanästhesie am NFKL möglich

 

 

4.2.2       Durchführung / Anwendung

- Pabal Mittel der 1. Wahl bei fehlender Kontraindikation (s.u.)

- präoperativ gewünschtes / geeignetes Kontraktionsmittel kommunizieren, sich auf Kontraindikationen hinweisen !

- Applikation: 1 Ampulle (1ml) à 100mcg Carbetocin (Pabal®) nach   Kindsentwicklung unverdünnt langsam über 60 sec i.v.

- Einmalgabe! Nicht wiederholen

- Kein Syntocinon-Dauertropf post sectionem nach Gabe von Pabal

- Ampullen im Kühlschrank bei +2-8°C aufbewahren

 

 

4.2.3       Pharmakokinetik / Wirkung

- Wirkungseintritt nach ca. 1,5 Minuten, HWZ 40 Minuten

- Carbetocin ist ein langwirkender Oxytocin-Agonist und bindet selektiv an die Oxytocin-Rezeptoren im Myometrium.

- Eine einmalige intravenöse Dosis von 100 µg Pabal® (Carbetocin) nach der Geburt ist ausreichend, um adäquate uterine Kontraktionen zu erreichen.

- Wirksamkeit entsprechend ca. 50 IE Oxytocin

 

 

4.2.4       Aufklärung

im Rahmen der üblichen mündlichen Patientenaufklärung über Einsatz von Kontraktionsmitteln

Stillen: keine Besonderheiten

 

 

4.2.5       Kontraindikationen

- Darf nicht zur Induktion der Wehentätigkeit verwendet werden.

- Überempfindlichkeit gegenüber Carbetocin oder Oxytocin.

- Schwere Nieren- oder Lebererkrankungen.

- Präeklampsie oder Eklampsie / HELLP

- Schwerwiegende Herz-Kreislauf-Erkrankungen.

- Epilepsie

 

 

4.2.6       UAW

vergleichbar mit Oxytocin

- Hypotonie nach Gabe ! + Reflextachykardie

- cave antidiuretische Wirkung: Hyponatriämie möglich (bes. bei grösseren begleitenden Infusionsvolumina)

- Übelkeit, Erbrechen

- Gesichtsrötung/Hitzewallung

- Pruritus

 

 

4.2.7       Vorgehen bei ungenügender Uteruskontraktion nach Pabal

- Keine weitere Gabe von Syntocinon oder Pabal sinnvoll

- Atonie / Blutung =>   Nalador i.v.

- ggf.: fehlende Tonisierung ohne Blutung oder leicht vermehrte Blutung

  =>  1000 mcg Misoprostol (Cytotec®) rektal

- Kombination Misoprostol / Nalador vermeiden, Nalador ist bei Blutung zu bevorzugen, hat aber erhöhtes UAW-Profil
